29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a.) Service Spring Corporation located at 4370 Moline Martin Road Millbury, OH; On or about February 7, 2012, the employer did not assure employees were protected from unexpected startup of the Auto #3 Machine 	in the Auto Area. Employees were exposed to amputation and crush hazards while untangling wire in the adjacent wire tower.     per ISA instance B deleted: b.) Service Spring Corporation located at 4370 Moline Martin Road Millbury, OH; On or about February 7, 2012, the employer did not assure employees were protected from unexpected startup of the Auto #1 Machine 	in the Auto Area. Employees were exposed to caught by hazards while threading wire through the guide rollers to setup new wire.

Service Spring Corporation located at 4370 Moline Martin Road Millbury, OH; On or about February 7, 2012, the employer did not assure employees were protected from the rotating tooling on the Coning Machine in the Middle UPS Area. On January 6, 2012, an employee received a partial amputation of the right ring finger while attempting to place a cone on the end of a rotating tool.    b.) Service Spring Corporation located at 4370 Moline Martin Road Millbury, OH; On or about February 7, 2012, the employer did not assure employees were protected from an open sided cabinet containing feeder wheels on Auto #1 in the Auto Area. Employees were exposed to caught by and crush hazards while operating Auto #1.

Service Spring Corporation located at 4370 Moline Martin Road Millbury, OH; On or about February 7, 2012, the employer did not certify that a hazard assessment was performed to address the chemical and physical hazards found in the workplace.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Service Spring Corporation located at 4370 Moline Martin Road Millbury, OH; On or about February 7, 2012, the employer did not assure employees were protected from the sharp edges of wire and metal springs. Employees were exposed to hand and forearm lacerations while handling metal wire and springs.
