29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Tiffin Scenic Studios, Inc., Tiffin, Ohio: On or about February 7, 2012, the employer did not assure that written energy control procedures were developed and implemented to control hazardous energy such as mechanical and pneumatic when employees were engaged in operations such as, but not limited to, job setups on the Johnford ST-70 and the OmniTurn GT-75 CNC lathes. Employees were exposed to laceration and amputation hazards while engaged in these activities.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a) Tiffin Scenic Studios, Inc., Tiffin, Ohio: On or about February 7, 2012, the employer failed to train employees to isolate energy sources and render equipment inoperative before assigning employees to perform maintenance and servicing work where unexpected startup and energy releases exposed employees to the hazards of being caught-in or crushed by equipment, such as, but not limited to job setups on the Johnford ST-70 and the OmniTurn GT-75 CNC lathes. Employees were exposed to laceration and amputation hazards while engaged in these activities.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Tiffin Scenic Studios, Inc., Tiffin, Ohio: On or about February 7, 2012, the employer did not ensure that a rotating cutting blade was properly guarded on the Kysor Johnson manufactured (Serial #13662) band saw, in that an unused portion of the blade on the operators side was unguarded, exposing employees to amputation hazards. b) Tiffin Scenic Studios, Inc., Tiffin, Ohio: On or about January 24, 2012, employees operating the Johnford ST-70 CNC lathe were exposed to injuries from the machines rotating parts and pinch points in that employees bypassed the sliding door guard and accessed the hazard of the machine while it was in operation. The sliding door guard was equipped with an interlock, but it had been disabled. Employees were exposed to caught-on and amputation hazards while engaged in this activity. c) Tiffin Scenic Studios, Inc., Tiffin, Ohio: On or about February 7, 2012, employees operating the OnmiTurn GT-75 CNC lathe were exposed to injuries from the machines rotating parts and pinch points in that employees bypassed the sliding door guard and accessed the hazard area of the machine while it was in operation. Employees were exposed to caught-on and amputation hazards while engaged in this activity.
