29 CFR 1910.178(m)(5)(iii): When the operator of an industrial truck is dismounted and within 25 ft. of the truck still in his view, the load engaging means was not be fully lowered, controls were not neutralized, and the were brakes set to prevent movement:    (a) Near the pad area of building 861 on or about February 9, 2012, the parking brake was not set on a Hyster forklift, model 240 XL-Mil.  The lift was parked with the motor off at the end of a 20 foot long shipping container.  The operator was off the forklift to obtain medical chests from the far end of the container and to place them on a pallet on the forklift's tines.
