29 CFR 1910.244(a)(2)(iii): Loads were not cribbed, blocked or otherwise secured immediately after being raised with a jack: On or about December 13, 2011, at the facility, while working on a bus, an employee was exposed to crushing injuries when the bus they were working on fell when a jack failed to hold the load and the employee was beneath a part of the vehicle. The bus had not been cribbed, blocked or secure to prevent the load from falling in case of the hydraulic jack failing.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): North Lot, on two 110 volt orange extension cords, and the battery charger, the outer insulation was pulled out of the plug, allowing any strain to be placed onto the electrical wiring and connections. The cords were in use and were supplying 110 volt power to the battery charger.
