The employer did not ensure that respirators were cleaned and disinfected using the procedures in Appendix B-2 of this section, or procedures recommended by the respirator manufacturer, provided that such procedures are of equivalent effectiveness.     a. Maven Solutions, LLC located in Delta, Ohio: On or about February 10, 2012, the employer did not ensure the air supplied welding hood loose fitting respirator was clean and sanitary.  The respirator was covered in dust when the employer brought it out of the respirator storage cabinet.

29 CFR 1910.254(b)(4)(iv): Terminals for welding leads were not protected from accidental electrical contact by personnel or by metal objects i.e., vehicles, crane hooks, etc. Protection was not obtained by use of: Dead-front receptacles for plug connections; recessed openings with nonremovable hinged covers; heavy insulating sleeving or taping or other equivalent electrical and mechanical protection. If a welding lead terminal which is intended to be used exclusively for connection to the work is connected to the grounded enclosure, it was not done by a conductor at least two AWG sizes smaller than the grounding conductor and the terminal shall be marked to indicate that it is grounded.      a.
