29 CFR 1910.23(c)(3):     Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board.    a)  On or about February 6, 2012, Dukane Precast Incorporated did not maintain a standard railing adjacent to batch plant sand bins that are permit required confined spaces.  The batch plant bin number 2 wall adjacent to the access platform extended 27 inches above the platform without additional railing.     Abatement certification is required for this item in accordance with the rerquirements of 29 CFR 1903.19(c).

Abatement Documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.146(d)(9):     Develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and for preventing unauthorized personnel from attempting a rescue;    a)   On or about February 6, 2012, Dukane Precast Incorporated did not implement procedures for immediately summoning emergency services upon discovery of an employee engulfed in sand within a permit required confined space.     b)  On or about February 6, 2012, Dukane Precast Incorporated did not implement procedures for preventing unauthorized co-workers from entering a permit required confined space and attempting a rescue of an employee engulfed in sand.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
