29 CFR 1910.22(a) : Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:     On or about September 17, 2013, did not ensure the wood shop was maintained kept clean and orderly.  The woodshop had wood dust built up around equipment an surfaces, thereby exposing employees to the hazards with fire.    DOCUMENTATION AND CERTIFICATION IS REQUIRED FOR THIS ITEM.

29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested:   Employees at Dukane Precast Incorporated, who are required to wear tight-fitting respirators, were not fit tested within one year of their last fit test.  Employees were last fit tested in February, 2012.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
