29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to items (a) through (d) of this section:      Batch Plant - On or about August 4, 2014, the employer failed to clearly identify all of the specific steps to be followed by employees to de-energize equipment and control the hazardous energy.  Employees were thereby exposed to the unexpected re-energization of equipment.

29 CFR 1910.147(c)(7)(i)(C): All other employees whose work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or re-energize machines or equipment which are locked out or tagged out.    Throughout Facility - On or about August 4, 2014, the employer failed to trained other employees that may be in the area where lockout tag out procedures are being performed, thereby exposing them to the re-energization of equipment.

29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through.    Batch Department - On or about August 4, 2014, a floor hole did not have a secure cover in place, thereby exposing employees to trip hazards.
