Acumulation of dust was visible on the horizontal surfaces of the shop such as but no limited to electrical conduit and overhead lights. Other areas of the worksite were also covered with dust generated from cutting and sanding the dried foam and the antimicrobial-treated dry wall. The condition was noted on or about 02/15/12. The conditoin was noted on or about 02/23/12.    c) Subzero Insulation and Refrigeratoins Technologies - An extensoin cord used to power a Makita 5007F Circular Saw did not have strain relief. The condition was noted on or about 02/23/12. 29 CFR 1910.134(h)(1)(i): Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in sanitary condition:     a) Subzero Insulation & Refrigeration Technologies - Employee helping the sprayer was required to use tight fitting full face air purifying respirator while spraying 2-part isocyanate insulation foam, and painting the retrofitted refrigerated vans. The employer permited the employees to wear a respirator that was not clean and desinfected. The condition was noted on or about 02/15/12.      Subzero Insulation & Refrigeration Technologies was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 315478537, Citation Number 1, Item Number 6, Group a, issued December 12, 2011, with respect to a workplace located at Jacksonville Florida. The citation became final order on January 5, 2012.    NOTE:  CERTIFICATION AND DOCUMENTATION OF ABATEMENT MUST BE SUBMITTED FOR THIS CITATION ITEM AS REQUIRED BY 29 CFR 1903.19.
