(a)  The Employer entered a check mark in column J (Other Recordable Injuries) for an injury involving a fractured foot which occurred on 12/1/11 and resulted in one lost work day instead of in column H (Days Away From Work).

29 CFR 1904.32(b)(3):   A company executive must certify that he or she has examined the OSHA 300 Log and that he or she reasonably believes, based on his or her knowledge of the process by which the information was recorded, that the annual summary is correct and complete.    (a)  On and prior to Febraury 15, 2012, the 2011 summary was not filled out and certified as correct and complete.
