29 CFR 1910.212(a)(1):     One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    a. In the molding department, employee(s) were exposed to the point of operation on the mill machine due to the air cylinder not being guarded when activated to hold the part in place.

29 CFR 1904.4(a):     Basic requirement. Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that is work-related, is a new case, and meets one or more of the general recording criteria:    a. On or about February 9, 2012, a recordable injury occurred when an employee received a smashed finger on the right hand from a mill machine and the injury was not recorded on the employers injury/illness records.
