29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: Frequent and regular inspections of the jobsite located at 49 Airport Road, St. Louis, MO, 63135, were not performed by a competent person who was capable of identifying the fall hazard during the inadequate implementation of a controlled decking zone (CDZ). The CDZ was not clearly marked while deckers were exposed to a 21 foot fall hazard as they actively decked the north phase of the building. After the deckers reestablished the CDZ, the warning tape did not limit the area where they were unprotected by conventional fall protection. The tape demarcating the CDZ extended the entire length of the approximately 166 foot wide structure.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: At the construction site located at 49 Airport Road, St. Louis, MO, 63135, an employee was observed working on the east side of the building in a JLG Aerial Lift, #W6312, without a personal fall arrest system (PFAS). The employee was approximately twenty-one feet above ground during the metal decking installation and was assisting to secure the metal building insulation.

1926.760(c)(3): The boundaries of a CDZ were not designated or clearly marked. The CDZ was more than 90 feet (27.4 m) wide and 90 (27.4 m) feet deep from any leading edge. The CDZ was not marked by the use of control lines or the equivalent: At the jobsite located at 49 Airport Road, St. Louis, MO, 63135: a) Deckers were installing metal decking without clearly marking the controlled decking zone. The control lines initially installed to demarcate the CDZ between support stanchions were down lying on the west side of the metal deck near the stanchions placed at the west side of the structure. b) The deckers reestablished the CDZ parallel to the leading edge by extending warning tape the entire length of the approximately 166 foot wide structure using supporting stanchions.
