The number of days away from work in Column K, case number 3, indicates that the employee was away from work for days, as well as the total number of the days away from work in Column K does not match the total of all cases with days away from work for the year. No additional abatement information is needed for this item.

The descriptions in Column F, case number 1, lacked the required information including, but not limited to, the object or task that caused the injury. B. Summers Rubber Company, located at 1223 Lima Avenue, Findlay, OH: On or about February 15, 2012, the employer did not provide the required injury or illness description on the OSHA 300 Log for the calendar year 2012.
