29 CFR 1904.7(b)(3):     The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent:    Summers Rubber Company, located at 1223 Lima Avenue, Findlay, OH: On or about February 15, 2012, the employer did not ensure that the correct number of calendar days away from work in the column K was recorded on the OSHA 300 Log for the calendar year 2011. The number of days away from work in Column K, case number 3, indicates that the employee was away from work for  days, as well as the total number of the days away from work in Column K does not match the total of all cases with days away from work for the year.    No additional abatement information is needed for this item.

29 CFR 1904.29(b)(1):     A log of all recordable work-related injuries and illnesses (OSHA 300 or equivalent), was not completed in the detail as required by the regulation:    A.	Summers Rubber Company, located at 1223 Lima Avenue, Findlay, OH: On or about February 15, 2012, the employer did not provide the required injury or illness description on the OSHA 300 Log for the calendar year 2012. The descriptions in Column F, case number 1, lacked the required information including, but not limited to, the object or task that caused the injury.    B.	Summers Rubber Company, located at 1223 Lima Avenue, Findlay, OH: On or about February 15, 2012, the employer did not provide the required injury or illness description on the OSHA 300 Log for the calendar year 2012. The descriptions in Column F, cases number 2, lacked the required information including, but not limited to, the object or task that caused the injury.    No additional abatement information is needed for this item.
