29 CFR  1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  On or about October 8, 2024, Summers Rubber Company  did not ensure certification of all powered industrial vehicle operators at 12555 Berea Rd Cleveland, OH 44111 to include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.

29 CFR  1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":  On or about October 8, 2024, the employer failed to ensure that an emergency exit door, located by the walk-in counter,  was marked and identified as an emergency exit so that employees knew where to go and exit in the event of an emergency.

29 CFR  1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color:  On or about October 8, 2024, the employer failed to ensure that an emergency exit door (located in the back of the warehouse) had an illuminated exit sign located above the door so that door was identified as an emergency exit for employees to know where to go and exit in the event of an emergency.

29 CFR  1910.37(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge.  A device such as a panic bar that locks only from the outside is permitted on exit discharge doors.  On or about October 8, 2024, the employer failed to ensure that an emergency exit door located in the back of the warehouse  was able to be opened without tools or special knowledge in that the exit door was locked with a barrel bolt lock.
