Pearl St.,Buffalo, NY: Employees were exposed to the potential of stepping into two rectangular floor holes which were left uncovered. The dimensions of the holses were approx. 16 in. wide x 6 ft. long x 14 in. deep, and 12 in. wide x 8 ft. long x 14 in. deep. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever was less, were not equipped with one stairrail system along each unprotected side or edge: a) On or about 02/14/12, at the corridor stairs in front of the entranceway to Apartment 106 on construction site, 189 N. Pearl St., Buffalo, NY: Employees were exposed to a potential fall of up to 2 ft. 7 in. from both unprotected sides of the stairway. No stairrail system was present on either side. NO ABATEMENT CERTIFICATION REQUIRED
