On or about 02/02/12, at Nazareth Collast Avenue, located on 4245 East Avenue, in Rochester, NY., an electrical cord (ROMEX) had been routed through an a metal pipe entering a service trailer without protecting the jacket or insulation of the cord.

Electrical equipment shall not be used unless the manufacturer's name, trademark, or otherdescriptive marking by which the organization responsible for the product may be identified is placed on the equipment and unless other markings areprovided giving voltage, current, wattage, or other ratings as necessary. The marking shall be of sufficient durability to withstand the environmentinvolved. On or about , 02/02/12, at Nazareth College, located on 4245 East Avenue, in Rochester, NY., an electrical service panel feeding power to equipment and site trailers was not labeled indicating voltage and or ratings.
