29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:      (a)  The periodic inspection of the energy control program was not properly conducted, in that it did not include a review of each machine's energy control procedures, was not performed by an authorized employee, and was not conducted with each authorized employee.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(7)(i):  The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:      (a)  Employee training on the energy control program was inadequate, in that it did not include training for employees on when and how to lock out the Poreba Machine lathe, the LeBlond Machine Lathe, and the LeBlond Regal Machine Lathe prior to doing setups, that included adjusting and replacing the chucks, adjusting and replacing the drills, adjusting the jaws, and wiping down the chuck.    (b)  Employee training on the energy control program was inadequate, in that it did not include training employees on how and when to lock out the Mazak CNC Mill, prior to climbing into and cleaning the machine at the end of the shift.    (c)  Employee training on the energy control program was inadequate, in that it did not include training employees on when and how to lock out the Kastos bhs Hacksaw prior to changing the blade.    (d)  Employee training on the energy control program was inadequate, in that it did not include training employees on when and how to lock out the DoAll Vertical Band Saw prior to changing the blade.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(5)(i):  Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:      (a)  Employees were not provided with locks or tags to attach to the electrical energy isolating device, prior to setting up the Poreba Machine lathe, the LeBlond Machine Lathe, and the LeBlond Regal Machine Lathe, including adjusting and replacing the chucks,  adjusting and replacing the drills, adjusting the jaws, and wiping down the chuck during setups.    (b)  Employees were not provided with lock or tags to attach to the electrical and pneumatic energy isolating devices prior to climbing inside and cleaning the Mazak CNC Mill at the end of the shift.    (c)  Employees were not provided with locks or tags to attach to the electrical energy isolating device prior to changing the blade on the Kastos bhs Hacksaw.    (d)  Employees were not provided with locks or tags to attach to the electrical energy isolating device prior to changing the blade on the DoAll Vertical Band Saw. 29 CFR 1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.:    (a)  Employees operating the LeBlond Machine Lathe and the LeBlond Regal Machine Lathe, were exposed to the unguarded chucks.    (b)  Employees operating the Famous Line Vertical Band Saw were exposed to 4-1/4 inches of unguarded blade.    (c)  Employees operating the Tannewitz Vertical Band Saw were exposed to 3-1/4 inches of unguarded blade.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
