If an employee's hearing test (audiogram) reveals that the employee has experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level is 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS, you must record the case on the OSHA 300 Log. On or about February 22, 2012, the employer did not record the following workplace injuries and/or illinesses on the OSHA 300 Log certified on January 30, 2012 for the calendar year of 2011. a) On or about March 8, 2011 a recordable illness occurred when an employee received a Standard Threshold Shift (STS) in the right ear; however, the employer failed to record the illness on the OSHA 300 Log for the calendar year of 2011. b) On or about March 8, 2011 a recordable illness occurred when an employee received a Standard Threshold Shift (STS) in the left ear; however, the employer failed to record the illness on the OSHA 300 Log for the calendar year of 2011. c) On or about March 8, 2011 a recordable illness occurred when an employee received a Standard Threshold Shift (STS) in both the right and left ear; however, the employer failed to record the illlness on the OSHA 300 Log for the calendar year of 2011.
