a) Storage platform located above the employee restroom, employees accessing the platform were exposed to a ten (10) feet fall to the concrete floor below because there were no guard rails in place. There were obsolete tools and hydraulic parts situated on this platform. b) Storage platform located above the owner's office, employees accessing the platform were exposed to a ten (10) feet fall to the concrete floor below because there were no guard rails in place. There was a dog box, a blue print cabinet, electric boxes, Gaylord boxes and old tooling stored on this platform. c) Storage platform located above the supervisor's office and employee breakroom, employees accessing the platform were exposed to a ten (10) feet fall to the concrete floor below because there were no guard rails in place. There was ventilation pipe, insulation, hydraulic power units, and wood situated on this platform.
