29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  Employee(s) who performed work that was covered by the lockout standard, such as changing band saw blades and punch/dies on the ironworker, was not trained to lockout the machines.  Employee(s) were not trained on lockout work for an authorized employee.  Abatement documentation is required for this violation.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  A horizontal band saw and an ironworker were not locked out (energy isolated by a positive energy isolating devices, such as disconnects) during blade/tooling changes.  Abatement documentation is required for this violation.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  A lockout/tagout annual inspection was not conducted, such as to determine who was performing maintenance work that required lockout/tagout and what equipment maintenance procedures required lockout, as well as ensuring lockout/tagout specific procedures were developed and written.  (Note: the annual inspection is required to be documented.)  Abatement documentation is required for this violation.
