The employer had not issued enough locks for 13 employees to lock out both the electrical and pneumatic energy sources on equipment such as, but not limited to, the Portioner Machine prior to the teardown of the machines after production.

29 CFR 1910.147(c)(7)(i)(A): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. Employees who tear down the machines after production has ended on Second Shift had not received training as authorized employees on the energy control prodedures for the OMORI wrapping machine, Vemag machine and the Portioner machine.

Maintenance employees who perform energized electrical voltage testing on 480 Volts AC had not received training on what personal protective equipment to wear and when to wear insulated rubber gloves, FR clothing, eye protection, hearing protection, and head protection to protect themselves against electric shock and arc flash.

29 CFR 1910.147(c)(6)(i)(C): Where lockout is used for energy control, the periodic inspection shall include a review,between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected. The Company did not perform periodic inspections on an annual basis which includes evaluating authorized employees, maintenance employees performing lockout. American Foods Group was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 312550841,citation number1, item 2a, which was affirmed as a final order on March 17, 2009, with respect to a workplace located at 544 Acme St., Green Bay, WI 54301.
