1910.147(e)(2)(i)  The work area shall be checked to ensure that all employees have been safely positioned or removed.  On or about September 23, 2019, the employer did not ensure that operators on the backside of the Bone Carmon were adequately trained to remain a safe distance from the hopper after providing required visual and verbal confirmation that operators were a safe distance from the hopper prior to the manual release of the safety pins holding the hopper in place, which would permit the hopper to be tilted.
