29 CFR 1926.454(a):     The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a)  East end of building, northwest corner of scaffold, on or about 2-19-12:  Employees were using a scaffold system without the training necessary for employees to recognize the hazards associated with scaffold work.    Abatement Certification must be submitted for this item.

29 CFR 1926.1053(b)(5)(i):     Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder.    a)  East end of building, southwest corner of scaffold, on or about 2-23-12:  One employee was accessing a portable ladder that was at an angle of approximately 90 degrees, exposing the employee to a fall hazard of 14 feet.
