29 CFR 1926.451(e)(9)(i): The employer did not provide safe means of access for each employee erecting or dismantling a scaffold: a) At the jobsite, on or about 11/28/16: The employer did not ensure that employees assembling scaffolding did so with a safe means of access and did not prevent climbing the end frames and cross braces, exposing workers to falls of 12 feet to grade. QM Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.451(e)(9)(i), which was contained in OSHA inspection number 191979, citation number 2, item number 2, and was affirmed as a final order on 5/3/12, with respect to a workplace located at the Holiday Inn Express, 2-98 Wells Avenue, Utica, NY.
