29 CFR 1915.89(b):  The employer did not establish and implement a written program and procedures for lockout and tags-plus systems to control hazardous energy during the servicing of any machinery, equipment, or system in shipyard employment.    C/P Blue Attu:  On or about 2/12/2012, and times prior thereto, the employer did not have a locks and tags plus program implemented on the vessel that addressed the safe application of locking out headers and mechanical equipment, among other equipment; exposing employees to electrical and amputation hazards on the vessel while conducting these activities.

29 CFR 1915.89(c)(1):  The employer did not ensure that, before any authorized employee performs servicing when energization or startup, or the release of hazardous energy, may occur, all energy sources are identified and isolated, and the machinery, equipment, or system was rendered inoperative.    C/P Blue Attu:  On or about 2/12/2012, and times prior thereto, the employer did not ensure that energy isolation measures were utilized when changing out the Tokai header blade; exposing employees to electrical and amputation hazards on the vessel while conducting these activities.

(c)  Calendar Year 2011:  The NAICS or SIC was not included on the OSHA 300A.  In addition, the total number of cases with days away from work was listed as "unknown"  and did not relate to the number of injuries and illnesses.
