29 CFR 1910.212(a)(3)(ii):     The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a. Throughout facility - On or about February 14, 2012, employees were exposed to the point of operation when working with Tiromats with guard openings of 7/8ths of an inch, thereby exposing employees to amputation type injuries.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.    a. Summer Sausage Packaging Department - On or about January 25, 2012, an employee assigned to operate the Tiromat 3000 did not affix a lock to each energy source, in order to remove summer sausage stuck on the die.  The employee suffered a severe hand injury due to the unexpected re-energization of the cutting blade.     b. 2nd Floor Carnation Room (S. Packaging) - On or about February 7, 2012, an employee assigned to operate theTiromat #3000, Serial #741-30071560 failed to affix a lock to each energy source, and insted used magnets to override the safety limit switch, resulting in amputation injuries.

29 CFR 1910.147(c)(4)(ii):     The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:    Throughout facility- On or about February 14, 2012, the employer had not identified Nitrogen as a hazardous energy source requiring lockout on the Tiromat machines.      Bridgford Food Processing Corporation was previously cited for a violation of this safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 311370118, Citation 1 Item 2b and was affirmed as a final order on March 6, 2008 with respect to a workplace located at 170 North Green Street, Chicago, IL 60607.    Bridgford Food Processing Corporation was previously cited for a violation of this safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 313937997, Citation 3 Item 2 and was affirmed as a final order on February 2, 2011with respect to a workplace located at 170 North Green Street, Chicago, IL 60607.

29 CFR 1910.147(c)(7)(i):     The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:    Throughout facility - On or about February 14, 2012, the employer did not provide effective training to authorized employees to include all of the elements in items (a) through (c), thereby exposing employees to the potential of unexpected re-energization of equipment.
