1910.212(a)(3)(iii): Special handtools for placing and removing material shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided. a. On or about April 9, 2021, the employer did not provide and ensure that employees utilized special hand tools to remove material from the Kramer-Grebe, Machine No. 741-3005/560 and other operating equipment. These tools are to be used during normal production and employees are to be trained on appropriate use.

29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Semi-dry Packaging Department 44 - On or about April 9, 2021, an employee was exposed to machine hazards associated with moving parts when clearing material from the Kramer-Grebe , Machine No. 741-3005/560 . The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], Lockout/Tagout device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
