29 CFR 1915.89(c)(7)(ii):  The employer did not ensure the coordination process included a lockout/tags-plus coordinator and a lockout/tags-plus log.    (a) C/P Blue North:  On or about February 14, 2012, and times prior thereto, the employer had not documented lockout and tags plus operations to demonstrate that safe procedures have been implemented since the standard came into effect.  In addition, the lockout tags plus coordinator was not identified in writing.

29 CFR 1910.23(a)(3)(i): Every hatchway and and chute floor opening shall be covered by a hinged floor opening cover of standard strength and construction equipped with standard railings or permanently attached thereto so as to leave only one exposed side. When the opening is not in use, the cover shall be closed or the exposed side shall be guarded at both top and intermediate positions by removable standard railings.    (a)  Steering Hatch:  On or about February 14, 2012, and times prior thereto, the steering hatch was left open while employees were working in the area; exposing employees to a fall hazard of approximately 7.5 feet to the metal floor below.     (b)  Reefer Deck Hatch:  On or about February 14, 2012, and times prior thereto, the reefer deck hatch was left open while employees were working in the area; exposing employees to a fall hazard of approximately 7.5 feet to the metal floor below.     (c)  Hydraulic Room Hatch:  On or about February 14, 2012, and times prior thereto, the hydraulic room hatch was left open while employees were working in the area; exposing employees to a fall hazard of 8 feet to the metal floor below.     Yakutat, Inc. was previously cited for violations of the Occupational Health and Safety standard or its equivalent standard 29 CFR 1918.35 which was contained in OSHA inspection number 307507517, Citation 1 Item 1, issued on March 15, 2011, with respect to a workplace located at Dutch Harbor Alaska.

29 CFR 1910.242(a):     General requirements. Each employer shall be responsible for the safe condition of tools and equipmentused by employees, including tools and equipment which may be furnished by employees.    Weather Deck:  On or about February 14, 2012 and times prior thereto, fiberglass mallets that were used to break ice on the weather deck had cracks that extended halfway up their handles.  This exposed employees to struck-by hazards from the mallet handle breaking while breaking ice.
