OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrical hazards in the engine room: F/V Ocean Prowler (Engine room): On or about February 15, 2012, and times prior thereto employees were exposed to electrical shock hazards due to an unguarded opening exposing energized busbars on Panel PP-5.

You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. F/V Ocean Prowler: On or about 2/15/2012, the employer had not recorded a lost work time injury involving an employee who injured his hand on 1/7/2012 and missed 7 days of work as resultant of the injury.
