29 CFR 1918.87(d): During elevator operation, each open deck that presented a fall hazard to employees was not effectively barricaded. Factory Deck, Freezer Elevator Hatch: On or about February 15, 2012 and at times prior thereto, the rope line utilized to guard the open side of the hatch measured 34 inches at its maximum height. This condition exposed employees to a 12 foot fall hazard.
