29 CFR 1926.453(b)(2)(v):     29 CFR 1926.453(b)(2)(v):  A body harness was not worn and a lanyard attached to the boom or basket when working from an aerial lift:    Two employees were in an elevated basket of an aerial lift without any type of fall protection.  The employees were exposed to a fall hazard.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.21(b)(2)    The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:    At the jobsite, two employees did not receive training in relation to fall hazards prior to performing plumbing work from the approximately 12 foot high elevated basket of an articulating boom type aerial lift.  The employees were exposed to a fall hazard.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
