29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required: a. In regards to citation 1, item 1, issued on November 24, 2015, the employer has failed to submit abatement certification and documentation within ten days of the abatement date, December 7, 2015. The violation read as follows: 29 CFR 1926.300(b)(4)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. Located at building 23 at DSCC, a 918 Ridgid 300 roll groover was operated with no guard at the point of operation, exposing employees to in going nip points. On or about July 30, 2015 an employee sustained an amputation to the index finger when it was pulled into the point of operation.
