29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 1. In the bag filling area, near packer station A, the plexiglass (plastic shield) used to protect employees from physical contact with the air actuated pinch tube was broken. The plexiglass was broken in the upper right hand corner and wrapped with gray duct tape. 2. In the bag filling area, near packer station B, the plexiglass (plastic shield) used to protect employees from physical contact with the air actuated pinch tube was broken (cracked). The plexiglass was broken in the middle and wrapped with gray duct tape.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) (LOCATION) (IDENTIFY EQUIPMENT) (STATE HAZARD(S) TO EMPLOYEE(S)) 1. Inside of the employer's conference room, underneath the large conference room table, employees (management) were exposed to electrical shock hazards by using a metal fourplex outlet box with knockouts as an extension cord to energize work equipment such as computers and their components.
