29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts and crushing hazards: (a) Extruding Department: Gloenco 135-Winder machine did not have the ingoing nip points created from the lay on roller contacting the winding roller guarded, exposing employees to caught-in and crushing hazard. (b) Converting Department: Sonic Sealers/Welders on RoAn #20,#30, and #60 exposed employees to crushing/pinch point and hazard as the sealer/welder head indexed down onto the plastic films to seal plastic films together. POLYFIRST PACKAGING, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, (29 CFR 1910.212(a)(1)) WHICH WAS CONTAINED IN INSPECTION #97597, CITATION NUMBER 1, ITEM NUMBER 2, AND WAS AFFIRMED AS A FINAL ORDER ON 11/16/11 IN RESPECT TO A WORKPLACE LOCATED AT 2261 INNOVATION WAY, HARTFORD, WI 53027.
