Drilling Site:  On or about February 15, 2012, a flexible power cable was run through a door on a junction box next to the stairway to mud pits which caused a pinch point on the cable.  This condition exposed employees to potential electric shock hazards.

This condition exposed employees to fall hazards of approximately 14 feet to ground below.      HELMERICH & PAYNE DRILLING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1910.23(c)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314162272, CITATION NUMBER 01, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON MAY 16, 2011, WITH RESPECT TO A WORKPLACE LOCATION AT:  DUNN A1-H, 2614 SR, RTE 49 EAST, COUDERSPORT, PA.
