At the project site on or about February 24, 2012, and at times prior thereto, eye protection was not used while using a DW 317 Jig Saw to cut an opening on the side of an aluminum door. Employee was exposed to flying aluminum chips/shavings and other foreign objects to the eye, thereby potentially resulting in cuts or scrapes on the cornea.

At the project on or about February 24, 2012, and at times prior thereto, there was no ground-fault circuit-interrupter protection or an assured equipment grounding conductor program when an extension cord was used to power a 110 VAC DW 317 Jig Saw.  The jig saw was used to cut a hole on the side of an aluminum door.
