29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. At the project site on or about February 24, 2012, and at times prior thereto, eye protection was not used while using a DW 317 Jig Saw to cut an opening on the side of an aluminum door. Employee was exposed to flying aluminum chips/shavings and other foreign objects to the eye, thereby potentially resulting in cuts or scrapes on the cornea.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. At the project on or about February 24, 2012, and at times prior thereto, there was no ground-fault circuit-interrupter protection or an assured equipment grounding conductor program when an extension cord was used to power a 110 VAC DW 317 Jig Saw. The jig saw was used to cut a hole on the side of an aluminum door.
