29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) At the project site on or about February 28, 2012, and at times prior thereto, the 28 foot long Werner aluminum extension ladder did not extend at least 3 feet above the roof as it extended about 6 inches above the roof. Employees used the ladder to access the roof in order to apply roof coating. Employees were exposed to a fall hazard of about 14 feet to the ground below.
