29 CFR 1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employees noise exposures equaled or exceeded an 8 hour time-weighted average sound level (TWA) of 85 dBA:    An employee, performing abrasive blasting, was found to be in excess of the action level for noise and the employer had not implemented an effective hearing conservation program.    a. An employee was exposed to continuous and intermittent noise at 231.8% of the action level or an equivalent sound level of approximately 96.1 dBA during the 445 minute sample period.    b. An employee was exposed to continuous and intermittent noise at 300.5% of the action level or an equivalent sound level of approximately 97.9 dBA during the 445 minute sample period.

29 CFR 1910.134(c)(1):  A written respiratory protection program that included the provisions in 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  Employees sandblasting are required to wear a sandblasting hood and a 3M 5101 tight-fitting, cartridge type, half mask respirator and the employees were exposed to inhalation hazards.  Air provided to the air supplied sandblasting hood was not protected from carbon monoxide infiltration.  A written respiratory protection program had not been established by the employer that contained:    Procedures for selecting respirators for use in the workplace;    (i)	Medical evaluations of employees required to use the respirators;    (ii)	Fit testing procedures for tight-fitting respirators;    (iii)	Procedures for proper use of respirators in routine and reasonably foreseeable emergency situations;    (iv)	Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators;    (v)	Procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators;    (vi)	Training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations;    (vii)	Training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use, and their maintenance; and     (viii)	Procedures for regularly evaluating the effectiveness of the program.
