29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv): a) Sewing Room - On or about February 29, 2012, employees were exposed to burn hazards due to a spliced extension cord used as permanent wiring for the light and fan at the Brother single needle lock stitch sewing machine. b) Sewing Room - On or about February 29, 2012, employees were exposed to burn hazards due to three extension cords used as permanent wiring for the light at the Wilcox Gibbs Surger. c) Embroidery Room - On or about February 29, 2012, employees were exposed to burn hazards due to an extension cord used as permanent wiring for the light on the embroidery machine (furthest from entrance near stairs). d) Embroidery Room - On or about February 29, 2012, employees were exposed to burn hazards due to an extension cord used as permanent wiring for the light on the embroidery machine (closest from entrance near stairs).

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): a) Sewing Room - On or about January 9, 2012 a maintenance employee was exposed to burn hazards while changing the ballast of a flourescent light without locking out the light switch that served as the method of deenergization. The energy source is 110V.
