29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    A: On 02/28/12 employees were conducting roofing activities (cleaning the roof surface), the employer did not ensure employees used fall protection systems/devices.  Employees were exposed to a fall of 13 feet.     B: On 02/28/12 employees were conducting roofing activites (detail work), the employer did not ensure employees used fall protection systems/devices.  Employees were exposed to a fall of 28 feet.

29 CFR 1926.451(f)(15):     Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph:    On 02/28/12 during roofing activities, the employer did not ensure employees did not place a ladder on top of a baker scaffold to gain access to another level.
