29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached: On or about February 28, 2012, during construction activities including but not limited to roofing work on a low sloped roof, the employer did not ensure employees were protected from fall hazards approximately 24 feet by ensuring each employee was attached to an individual anchor point capable of supporting 5000 pounds per employee.
