29 CFR 1926.452(m)(1):     Crawling boards shall extend from the roof peak to the eaves when used in connection with roof construction, repair, or maintenance.    On January 31, 2012, an employee was installing siding while working from a "Chicken Ladder" (crawling board) that did not extend from the roof's eave to the peak, leaving 3-4 feet of roof without safe access available.  This would make access onto the "Chicken Ladder" (crawling board) difficult, thereby exposing the employee to a fall hazard of approximately 10 feet from the eave.

29 CFR 1926.451(g)(1)(iii):     Each employee on a crawling board (chicken ladder) shall be protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board;      a) On January 31, 2012, an employee was installing siding from a chicken ladder without the use of fall protection or a grab line, thereby exposing the employees to a fall hazard from 10 - 20 feet.

29 CFR 1926.503(b)(1):     The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.    a)  At the site, employees were not provided fall protection training to perform work at elevated heights which increases the likelihood of a fall.
