29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: (a) Beechy Builders, LLC on or about April 05, 2012, at the address of 233 W. Church St. Marion, OH: The employer failed to ensure an employee working from a MEC 2633ES scissor lift was adequately protected from falls. The employee was permitted to utilize an extension ladder atop the scissor lifts elevated platform to reach structural steel member near the ceiling of the structure. While working from the ladder in this condition the employee was exposed to an approximate 33ft fall to the ground below.

29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift: (a) Beechy Builders, LLC on or about April 05, 2012, at the address of 233 W. Church St. Marion, OH: The employer failed to ensure employees working from the Genie model # S-60 aerial lift were adequately protected from falls. Employees were not clipped off to the basket of the stated aerial lift while the basket was elevated. The employees were exposed to an approximately 18 ft fall to the ground below.
