29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. a. The floor on the outside the trash compactor was littered with debris and also ice accumulates covering up holes or damaged concrete. Employees could easily slip, trip, or fall in these openings.

29 CFR 1910.22(a)(3): To facilitate cleaning, every floor, working place, and passageway shall be kept free fromprotruding nails, splinters, holes, or loose boards. a. The floor in front of the compactor (inside the building) was not kept in good repair. The holes range from .5 inches to 1 foot 5 inches in depth. The length varies from 10 inches to 4 feet 4 inches.

29 CFR 1910.22(b)(1): Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repairs, with no obstruction across or in aisles that could create a hazard. a. The floor by the outside trash compactor was not kept clear of debris and in good repair. Employees could slip, trip, and fall due to the debris and the ice on the floor. b. The floor inside of the trash compactor could create a slip, trip, and fall hazard due to uneven floors. The damage on the floor varied between 1/2 inch to 3 inches deep and the length varied between 3 feet 10 inches and 9 feet.

29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk was not guarded by either a standard railing with toeboard on all exposed sides or a floor hole cover of standard strength and construction. While the cover is not in place, the floor hole was not constantly attended by someone or shall be protected by a removable standard railing. a. The area outside of the compactor by the driveway contained 1 floor hole. An employee could easily get their foot caught in the floor hole and break a foot since the depth is 1 foot and 4 inches and the length is 8 inches by 11 inches.
