29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity: At the project site on or about March 5, 2012, and at times prior thereto, a grounded conductor (extension cord) was attached to receptacle # 2 of a Portable Power Distribution Unit (Spider Box), model number 8706GU, serial number, 41333. Receptacle #2 was identified as having reverse polarity. The Spider Box was used to provide electrical power to temporary lights and portable tools at the project site.

The Spider Box was used to provide electrical power to temporary lights and portable tools at the project site.
