29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: On 03/02/2012, employer did not have a continuing hearing conservation program instituted for employees who were exposed to noise levels exceeding the 8-hour time weighted average (TWA) of 85 dBA: a) Chip & Spool employee, at bench #17 was exposed to noise at a TWA sound level of 97.4 dBA over a 370 minute sample time. b) Chip & Spool employee, at bench #10 was exposed to noise at a TWA sound level of 94.1 dBA over a 349 minute sample time. c) Chip & Spool employee, at bench #2 was exposed to noise at a TWA sound level of 98.6 dBA over a 377 minute sample time. d) Snub Wheel employee, at stand grinder #5 was exposed to noise at a TWA sound level of 96.1 dBA over a 324 minute sample time. All provisions of 29 CFR 1910.95(c) through (n) must be covered in a hearing conservation program. Key elements include, but are not limited to the following: 1) Noise level monitoring 2) Audiometric testing 3) Wearing of hearing protectors 4) Training program 5) Providing copies of standard to employees 6) Recordkeeping ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.

29 CFR 1910.95(g)(1): The employer shall establish and maintain an audiometric testing program as provided in this paragraph by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels. Employees who were exposed above the 8-hour time weighted average (TWA) of 85 decibels in accordance with this paragraph of the Occupational Noise Exposure standard were not given baseline and or annual audiograms. ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED
