29 CFR 1910.178(a)(4):     Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    A Clark forklift Model Number CCS20MB Type LPG was not equipped with a seatbelt.  The factory plate states the truck meets ANSI B56.1 1969/1975 which would require seatbelts from the factory.  Therefore, the forklift was modified with the removal of the seatbelt without manufacturer's written approval creating a crushed-by hazard for employee(s) during a rollover. 29 CFR 1910.305(a)(2)(x):     Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.    a)  Flexible cord was not protected from abrasion for the tilters used at stand grinders two and six.  The cord was on the floor with a piece of angle iron screwed over it protecting it.  The cord was exposed to abrasions from the angle iron and could potentially lose its protective sheathing exposing employee(s) to electric shock.      b)  A conduit fitting located above stand grinder two was not connected putting strain and abrasion on the conductors it is intended to protect.  The conduit was coming down from the ceiling, placing added strain on the conductors.  The conductors protecting sheathing could be worn off exposing employee(s) to electric shock.    c)  A control for a tilter utilized near stand grinder two had been run through a fitting and hung on the flexible control.  The flexible control cord was not protected against abrasion and it could lose its protective sheathing, thus exposing employee(s) to electric shock.
