29 CFR 1910.147(d)(4)(i):     Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    On or about March 2, 2012, an employee of Arnold Magnetic Technologies, Inc., in Marengo, Illinois, engaged in maintenance activities on Coremaking Machine # 13 without isolating the Dimethylethylamine (DMEA) line.  While the employee was working, a significant release of DMEA, which may or may not have been related to the work, occurred on the regulator to the condenser tanks.  The employee did not affix a lockout device to an energy isolating device such as a valve and/or pipe blank.    DOCUMENTATION AND CERTIFICATION IS REQUIRED OF THIS ITEM.
