29 CFR 1926.304(i)(1):     Each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut:    Worksite:  Employees were using a Dewalt DW744 table saw to cut and rip trim work which was not equipped with a guard.

29 CFR 1926.451(f)(7):     Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.        Worksite:  The employer did not ensure that the scaffolding had been inspected by a competent person prior to allowing employees to begin working from it.

29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.            Worksite:  An employee who was working on a combination ladder jack/wall bracket scaffold at a height between 9 feet 4 inches and 18 feet above the ground was not provided with fall protection.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.        Worksite:  Employees who were walking and working on the first floor were exposed to fall hazards of up to 17 feet due to unguarded wall and window openings.
