29 CFR 1926.405(g)(2)(iv):     Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:      Worksite:  Employees were exposed to electrical shocks from extension cords which were damaged and had the outer protective coating pulled away near the plug which allowed strain to be placed on the terminal screws and internal components.
